Fractional Lasers Safe for Darker Skin
A small study suggests fractional laser resurfacing appears to be effective and safe for the treatment of acne scars in patients with darker skin types. The study performed out of Henry Ford Health System in Detroit, MI involved 15 patients, 12 of whom who were African American with skin types IV-V. Subjects were treated with a fractional laser for a total of five monthly treatments. Results showed a significant improvement in acne scarring.
Patients with darker skin types are more prone to post inflammatory pigmentation or PIH after various cosmetic treatments such as chemical peels, dermabrasion, and laser resurfacing. The new fractionated lasers treat a fraction of the skin leaving uninjured tissue around the treated area to allow for rapid healing and less risk of adverse events such as PIH.

Eyelid Protection
You are probably not aware the the eyelid region is one of the most common sites for nonmelanoma skin cancers. Skin cancer on the eyelid can cause significant disfigurement due to the complicated surrounding anatomy.
The most common signs of skin cancer on the eyelid include an area of skin that has a scar like appearance, a mass, an ulceration or sore and a red spot.
Sunglasses and even eyeglasses can provide significant protection from the sun although sunglasses provide more. In addition to protection against skin cancer of the eyelid, sunglasses also protect against cataracts and skin cancer around the surrounding eye area. Try not to wear your sunglasses too far out on the bridge of the nose as that will increase the amount of UV light that reaches the eye.

Melasma News
Two studies recently confirmed that combination treatments work best for melasma, an approach The Kest Clinic has been using for years.
The first study involved the use of a triple combination cream plus intense pulsed light or IPL treatments to treat moderate to severe melasma. Results showed that sequential treatment involving IPL and the triple combination cream provides significantly better reduction in melasma than IPL alone.
A second study found that patients with moderate to severe photodamaged skin who underwent intense pulsed light treatments had better results when they used hydroquinone/tretinoin topical products.

The Real Cause Behind Aging Skin
Live off the press from the Annual American Academy of Dermatology meeting, new insight clarifying the direct causes and mechanisms of aging skin. Basically, there are two causes of aged, wrinkled skin: time and UV radiation. Collagen being the key component in both causes.
Think of collagen as the support scaffolding for your skin. Special cells in the dermal layer of your skin are stimulated to create collagen by the stretching and mechanical stress in young undamaged skin. Collagenase, an enzyme that hastens the breakdown of collagen, is almost completely absent. The result is firm, taught, smooth, youthful skin.
UV radiation begins the wrinkle cascade of collagen breakdown by causing solar wounding of the skin and oxidative stress. However, even skin that has never been exposed to sunlight shows signs of aging as proteins become oxidized and collagen strands are broken. Time, or age related wrinkles tend to be smaller and finer than sunlight related wrinkles however the mechanism is the same, collagen breakdown.
So what can we do to help prevent all this collagen breakdown? Right now two of the best solutions are: retinoids and fillers.
Retinoids such as Retin-A, Tazorac®, Differin®, Avita, Renova etc . . . have been proven to inhibit collagenase production and enhance collagen production.

What's New in Acne Treatment
Great strides have been taken in the field of acne treatment. New to our armentarium are three topical (medication that is put on top of the skin) agents.

Most Commonly Asked Questions About Hair and Eyebrow Restoration
1. Does removing hair from the back or sides of the scalp leave a gap?
The scalp is very elastic which allows the skin to be sutured back together with very little tension. The end result is typically a 1-2mm scar, which is completely hidden within the patients hair. Usually, the only way the minimal scar would be visible, is if you shaved your head bald.
2. Does the transplanted hair require special care?
Your newly transplanted hair will go the same as your original hair. It can be colored, waved, or styled in any way to suit your lifestyle.
In the case of Eyebrow Restoration the hair will grow longer than typical eyebrow hair, so it will require periodic trimming.

3. What is Follicular Unit Transplantation?
Follicular Unit Transplantation is the standard for Hair Restoration today. We transplant individual follicles in the same way they naturally occur, which is as 1, 2 , and 3 hair follicular units. This way we can recreate an individuals hair in the same way it naturally occurs, which makes the results completely natural and undetectable. Using follicular units also allows us to place the follicles closer together which allows us to create excellent density.
4. Why do the old plugs look so unnatural? Can anything be done to make them more natural looking?
The old “plugs” looked like “dolls hair” because they were grafts consisting of 20-30 hairs compared to the typical 1-3 hair grafts we now use exclusively. We can make them look more natural by a variety of ways which include selectively reducing the size of the old grafts and recycling the hairs to different balding areas, and also placing newer follicular units (1-3 hair grafts) in between the larger old grafts to make them appear more natural looking.
5. How about Propecia, Rogaine, and the Hair Laser?
Propecia is indicated for most men and can definitely slow the progression of hair loss and in some cases regrow hair to a minor extent. Rogaine 5% for men and Rogaine 2% for women can also slow the progression of loss and in some cases regrow hair to a minor extent as well. I commonly advocate these medical treatments for hair loss as an adjunct to a Hair Restoration procedure. Rarely will these medical treatments alone provide the desired density of hair that can be achieved with Hair Transplantation. Laser combs and laser machines are commonly touted as a cure for hair loss, but in reality, there is very little scientific evidence to show that they have any significant impact on stemming the progression of hair loss or the regrowth of hair to any significant degree.

6. What about cloning of hair?
Unfortunately we are many years away from being able to clone hair in clinical practice. The science is rapidly evolving, but it appears likely to be decades away from actual use.

1. What causes hair loss?
Hair loss in men is most commonly caused by genetic factors which involve the hormone DHT which causes the loss of susceptible follicles in the areas affected by Male Pattern Baldness. The hair follicles on the sides and back are not susceptible to DHT, and therefore are not usually affected by DHT.
Hair Loss in women can be caused by a variety of medical issues, but most commonly is related to genetic factors. Hypothyroidism, Iron Deficiency, and Autoimmune Diseases are other common causes for hair loss in women.
Other causes of hair loss include previous surgical procedures on the face, which can result in hair loss in the frontal and temple regions, as well as unsightly scars, which can be covered with transplanted hair.
Eyebrow hair loss is most commonly a result of overzealous plucking. Other causes include Thyroid abnormalities and previous trauma.

2. Are Hair Transplants permanent?
In Hair Restoration the transplanted hair comes from the “safe donor area”( the sides and back of the scalp). Since it is not sensitive to the balding process it is permanent hair. It retains it genetic characteristics and will continue to grow as normal hair for the rest of your life.
3. How many procedures and how much grafts will I require?
How much work you will require depends on many issues such as the severity of your hair loss, the amount of hair loss you will have over time, and the density desired. There are other factors as well which include skin to hair color contrast, caliber of hair, and character of hair (straight vs. wavy). In consultation, Dr, Dauer can explain where your loss may end up, what to expect in the future, and how many grafts are required for your desired density.
4. Is the procedure painful?
The discomfort associated with Hair Restoration is fairly minimal. The entire procedure is done awake under local anesthetic. The only pain during the procedure is associated with the few injections to numb the areas involved. After the numbing injections the procedure is relatively pain free. Post procedure there is a little tightness in the “donor area” for 1-2 days, but it is usually handled easily with over the counter pain medication or prescription pain medication we provide our patients. Our procedure suite is equipped with a large screen TV, along with DVD, and cable, so our patients can relax in comfort while undergoing their procedure.
5. How long after the procedure may I go back to work?
Post procedure the grafts appear like small grains of sand, which represent small scabs, for about a week. After a week most of these scabs fall out and there is no visible sign of the procedure. If the pre- existing hair is long enough it can usually be creatively styled to cover up the affected areas immediately. If not, the patient may wear a hat for the first week for coverage. Patients commonly return to work 1-2 days post procedure.
